New Patient Registration
If you live within our practice area (see above tab) you are welcome to register with us. If you have your medical card, please bring this into the surgery and hand it in at reception to register. Alternatively if you do not have this, please ask for a New Patient Registration Pack and you will be given some forms to fill in and return. The receptionist will also make an appointment with the nurse for a 'New Patient Health Check'.
It is very important that you attend this appointment as it will enable us to note down any important health problems that you may have (it can often take several weeks for your medical records to arrive at the practice). You will also need to bring a sample of urine with you, and a urine pot will be given to you at the time of registration.
The health check will involve a blood pressure check, blood sample being taken, a dipstick of your urine sample, recording of your height and weight and some lifestyle questions. You will also need to complete a 'New Patient Questionnaire' (given in the Registration Packs) to bring with you to the appointment so we can record any previous illnesses or problems.
It is a condition of joining the Surgery that all eligible patients must attend for a new patient check with the Practice Nurse. If you do not attend, your registration may not be completed and you may have to register at an alternative Practice.
