Bolton Neighbourhoods
The needs of local people have been increasing- people are living longer with more complex health conditions- and this is placing significant demands on services. We want to provide the best care across our town, but we also know that needs can vary significantly across different communities. We have taken a neighbourhood approach to ensure we have a close connection to our communities, whilst being large enough to support multi-agency working.
Our approach divides Bolton into six neighbourhoods, each with a population between 30 – 55,000 people. Services based within the Neighbourhood form part of a wider system of Integrated Care- working with a range of partners, including Health and Care, Housing, Police, voluntary sector- and connecting to assets within our communities.
This approach ensures that everyone is working together around the needs of the neighbourhood, enables services to provide proactive care in a more personalised way and helps people to stay well for longer, as part of a wider approach to prevention and health improvement
